Paperwork & Documentation Requirements

At this stage, eligibility has been verified. The following steps must be completed in full before services can begin.

Required Identification Documents

All recipients (or parent/guardian for minors) must upload the following into the Recipient Portal:

  • A clear copy of a valid State Driver's License or State ID
  • A clear copy of the Louisiana Medicaid Insurance Card
  • A clear copy of the Social Security Card

All documents must be current and legible.
